Nominate a non-relative (close family friend), or cousin to act as your approved care-giver (The person must be over the age of 21).

Complete Under 18 care arrangements form (pdf 191kb) |

Download (or complete form sent to you in under 18 package) the 'Parental/guardian authorisation for underage access' form (pdf 100kb), have your parents or guardian sign the form and provide the form to Monash University International Admissions with your acceptance and accommodation and welfare documents. |
| Step 3 |
Make appropriate arrangements for the completion of the Statutory Declaration Form (pdf 447kb) by the Care-giver in Australia and inform non-relative that they need to obtain a police clearance from the Victorian Police. |
| Step 4 |
Complete International Admissions Acceptance form (pdf 318kb) and provide payment |
| Step 5 |
Send the completed and appropriately signed forms to Monash University International Admissions office |
| Step 6 |
Obtain the signed Department of Immigration and Citizenship 'Education Provider Confirmation of Appropriate Accommodation/Welfare' (CAAW) form and your electronic Confirmation of Enrolment (eCOE) from Monash University International Admissions.
An eCOE will be issued after the required deposit and acceptance documents have been received. |

Submit both forms to the Department of Immigration and Citizenship with visa application form 157A. You will need to provide the CAAW form along with your eCOE and visa application form to the Department of Immigration and Multicultural Affairs (DIMA) before your student visa will be issued to you. |
