Submission Instructions

1. Submit Annual Report each anniversary of the approval date for all your current projects.
2. Submit Final Report at the completion of a project or data collection.
3. Requests for extension will be considered for periods at least 3 months prior to the end of the term of approval.
4. If the project involves multiple organisations and MUHREC is not the primary HREC and you have already submitted an Annual Report Form to the primary HREC, MUHREC will accept a copy of that report.

Terms and conditions for extension approval

1. Please note that all 2006-2008 projects have been approved for 5 years. Projects between 2001-2005 were approved for three years.
2. Requests for Extension must accompany or follow a completed Annual Report. 
3. Requests for Extension may be made when researchers have submitted a Request for Extension form prior to the end of term of approval and an Annual Report each year.
