Low Risk Research Applications

Form LR is intended to be used for all research where the foreseeable risk to participants is no more than one of discomfort. For example:

  • Questionnaires and/or surveys, anonymous or otherwise, online etc. involving non-sensitive topics
  • Observations, with or without consent

For  more information, refer to the National Statement on Ethical Conduct in Human Research 2007 Chapter 2.1: Risk and Benefit.

Submission requirements

Please note, you can submit Form LR at any time. There are no closing date requirements. Most Low Risk Applications are determined by way of an expedited process, not at Committee meetings.  We endeavour to provide a response within 2 weeks of receipt of your electronic submission.

 

 

IMPORTANT:

  1. Completing the checklist at the beginning of Form LR is mandatory. The Committee advise that you complete this checklist carefully and thoroughly before completing the form itself. If you answer 'Yes' to ANY of the items on the Checklist of the form pages 2 and 3, please stop filling in this form and use Form 1. If you answered 'No' to ALL of the items, please continue filling in Form LR.
  2. If another HREC is the Primary HREC, go to the multicentre page for more information.

 

Retrospective Form

 

 

Please forward:

1.            An original, completed, signed, hard copy of the application form and all other documents, e.g. explanatory statement, consent form, survey instrument, permission letter, advertisement for project etc to MUHREC, Room 111, Building 3e, Monash Clayton Campus

AND

2.           An electronic version of all of the above to muhrec@monash.edu

The e-copy of the application should be submitted via the Chief Investigator's Monash email account. 

The e-copy of the application form does not require signatures and should be in a word.doc format, other attachments may be in other formats, eg .pdf.
