Form HT is intended to be used for all researchwhere stem cells or samples of tissue, including blood and other body fluids, are collected from people in hospitals and other health care institutions, and in field research.

For further information, refer to the National Statement on ethical conduct in human research 2007 Chapters .3.4; 3.5 and 3.6

Submission Requirements

1. Four signed identical hardcopies: one (1) original + three (3) double sided photocopies containing the Form; Form P (Privacy Issues) and all other documents i.e. explanatory statement, consent form, survey instrument, permission letter, advertisement for project etc.
2. An electronic version of the application form + all relevant documents submitted to muhrec@adm.monash.edu.au
