Human Tissue Form application process

Human Tissue Form applications can be submitted at any time. They will be processed in order of receipt and will be allocated to the next available meeting. Your receipt email will include the reference number and the date of the meeting at which your project will be discussed.

The Committee's response will be emailed to all investigators listed on the project within 10 working days of the meeting date indicated on the receipt.  Please do not contact the office for progress reports within that time frame.
