Form 1: Human Ethics

Form 1 is intended to be used for all research where the foreseeable risk to participants is more serious than discomfort.

Please complete this form if your research involves the following:

 

  • Interventions and therapies
  • Human genetics and/or stem cells
  • Women who are pregnant and the human foetus
  • People highly dependent on medical care who may be unable to give consent
  • People with a cognitive impairment, an intellectual disability, or a mental illness
  • Aboriginal and Torres Strait Islanders
  • Research intended to study/expose illegal activity
  • Use of collected human data for a new purpose

 Important - If another HREC is the Primary HREC, go to the multicentre page for more information.

 

Submission requirements

 Please forward:

  1. Four identical, signed, hardcopies: (1) original + (3) double sided photocopies of the application form and all other documents, e.g. explanatory statement, consent form, survey instrument, permission letter, advertisement for project etc, to MUHREC, Room 111, Building 3e, Monash Clayton Campus, and

  2. An electronic version of all of the above to muhrec@monash.edu

The e-copy of the application should be submitted via the Chief Investigator's Monash email account. 

The e-copy of the application form does not require signatures and should be in a word.doc format, other attachments may be in other formats, eg .pdf.
