A condition of approval from the Monash University Institutional Biosafety Committee is that researchers submit an annual report on the activities covered by a Dealing.
Annual reports will be submitted online using the form below.
You must submit a report for each Dealing. Do Not submit multiple Dealings in one report.
Submission Instructions
The Principal Investigator, the first named person on the Dealing submitted to the IBC, should access the system using their Monash University Authcate username and password.
Enter the IBC Reference Number for the Dealing in the appropriate field. If you do not know what your Reference Number is please contact the Research Compliance Officer.
For changes to procedures, please provide sufficient details so that the IBC can assess whether the classification of the Dealing is still appropriate.
For changes to facilities, please provide details of the building, room and OGTR certification number.
