Biography

Putting research into costly falls on solid ground

Dr Lesley Day has a simple ambition of keeping more of us on our feet for longer. As the head of falls prevention research at the Monash Injury Research Institute (MIRI), she’s working to slow the steady growth in deaths and injuries from falls, which were the main cause of Victorian hospital injury admissions in 2009 and cost three times more to treat than road injuries.

For several years, Lesley has used a graph to explain the importance of her research to government ministers and bureaucrats. It’s a graph that everyone should see. 

It shows the number of days that hospital beds were occupied in Victoria in 2009, based on age and injury. Bed days due to falls begin rising dramatically at about age 50, become a cliff at about age 70 and peak at age 85. In that year seniors accounted for half the 42,346 admissions. And, of the $242 million spent on treating falls patients, $150 million was spent on treating seniors.

These figures don’t include deaths from falls or count the cost of getting people back on their feet after leaving hospital. Also, Lesley’s research has identified the need to pay more attention to seniors who visit hospital after a fall but aren’t admitted.

“Even if they don’t involve serious injury, falls among older people can lead to a cascade of events that shake their confidence and cause them to restrict their activities, increasing their risk of another fall – and the more often they fall, the more likely they’ll hurt themselves seriously,” she says.

One of Lesley’s PhD students tested the value of assessing and supporting fallers who visited but weren’t admitted to hospital. It found that a home visit from an assessor to identify risk factors, such as poor balance, vision or medication, and arrange referrals was not enough to reduce the risk of a second fall. Similar research suggests more active follow-ups are required.

Another of Lesley’s research projects identified the role of exercise in keeping us upright as we age.

“We know that exercise can prevent falls among older people, but they’re not easily motivated to exercise for this reason as they don’t think they’ll have a fall,” she says. “But we all want to stay independent, so we can promote exercise as a way to remain independent and also prevent falls.”

Lesley’s history of innovative injury research includes supervising a study of falls among jockeys, where her research student created the first national database of jockey injuries, and investigating farm injuries, which the Victorian Government drew on to frame laws that require tractors to have anti-rollover bars fitted.

Helping to save lives and cut injury is a major reward of her job, she says, which is helped by access to MIRI’s Victorian Injury Surveillance Unit, Australia’s most comprehensive injury database.

Lesley says good data is essential to identifying the real causes of injuries because, unlike disease, which requires an expert to explain, everyone can see an injury happen, and everyone can have an opinion on why it happened.

“This abundance of opinions has tended to distract from the research, what it’s actually saying, and from making decisions, based on the evidence rather than what people think.”

Lesley is also a deputy director of MIRI, which grew out of the Monash University Accident Research Centre, a world pioneer in road injury research and now one of several MIRI units. Her executive responsibilities include raising MIRI’s research profile.

“In the past, we’ve tended to publish more in reports and less in the peer-reviewed literature, so we’ve shifted the balance to demonstrate the rigour of our work and increase its accessibility.”

Lesley says the shift is important as MIRI’s broadened horizon includes injury outcomes and intentional injuries, such as suicides, Victoria’s main cause of injury-related deaths, and assaults, including on children, which Child Abuse Prevention Research Australia (now part of MIRI) investigates.

“We want to focus on all the big injury issues that concern governments and society, and we can now present ourselves as the place to come to solve them.”
