Inflammation and fibrosis in kidney disease
Kidney disease caused by type 2 diabetes is an emerging epidemic in the developing world. It accounts for 40 per cent of all cases, requiring dialysis or transplants as treatment. Yet many people are not aware that they may be in the early stages of the disease. Doctor David Nikolic-Paterson is helping to find new ways to identify and treat the damage wrought on our kidneys.
"There are many different types of kidney disease, which vary in both the underlying cause and in the speed with which they progress to end-stage kidney failure – at which point you require dialysis or a kidney transplant to survive," notes David.
"Our research is focused on understanding the disease mechanisms that drive progressive kidney damage, in particular inflammation and fibrosis, and to identify new therapeutic approaches in the treatment of this major health problem.
“We’re working in two areas. The first is basic research into the mechanisms of inflammation and fibrosis that underlie the development of many forms of kidney disease. The second area is translation of this basic research towards new clinical therapies.
"Our group has done a lot of work with a signalling pathway (termed JNK) which we have shown promotes both inflammation and fibrosis in different types of the kidney injury. Using a JNK inhibitor compound, we have found that pharmacologic blockade of this pathway is effective in experimental models of kidney disease, and we’re trying to translate these exciting findings into clinical trials.
"Another signalling pathway that we are working on (called c-fms) is required for the function of monocytes, a cell type involved in the immune response. Our previous work established that monocytes can cause acute renal injury."
Together with Dr Greg Tesch and Dr Frank Ma, David has been been able to block the action of monocytes and ease acute renal injury in several types of kidney disease, including kidney transplant rejection. These promising preclinical studies using a pharmacologic inhibitor of the c-fms pathway have potential for translation into clinical treatment for acute forms of renal injury.
A kidney transplant is the most effective treatment for patients with end-stage renal failure. There are many more patients awaiting transplants than there are available kidneys.
“Here at Monash Medical Centre we are doing around 75 transplants per year, and this has been increasing in part due to greater community awareness - with more people signing up to organ donation programs and improvements throughout the whole system.
"However, one area of concern is those patients who have pre-existing antibodies against potential donor kidneys, making them unsuitable as a recipient for many of the possible donor organs, for they may experience significant problems with rejecting the transplant.
"This is an important area in which I am working with Drs John Kanellis and Bill Mulley to examine the mechanisms of antibody-mediated rejection and developing the therapeutic options.”
