Privacy and Information consent statement
Information you provide when purchasing a Monash Community Card is collected for the primary purpose of administering the Monash Community Card program.
Purchasing a Monash Community Card implies consent to the University providing necessary personal data to associated internal and external organizations. Personal information from the Monash database (name, postal address, email address, ID number and campus details) may be disclosed to the following types of organisations:
- external printing firms for the explicit purpose of printing your details on your Monash Community Card.
- external mail distribution centres for the explicit purpose of sending your card, voucher booklet and welcome letter to your home address.
- external benefits provider(s) for the purposes of creating and managing your log in details, personalised online account and offering benefits.
- The Monash Community Card team for the purposes of emailing you with periodic updates of new services associated with the program. You may opt out at any time from receiving emails.
Non-identifiable aggregate data may be disclosed to the following types of organizations:
- Student organizations for the explicit purpose of tracking broad card usage patterns for reporting purposes and to inform the provision of future services and benefits associated with the Monash Community Card program.
If all of the information requested is not provided, it may not be possible for the University to process your Monash Community Card application. You have a right to access personal information that Monash University holds about you, subject to any exceptions in relevant legislation. If you wish to seek access to your personal information or inquire about the handling of your personal information, please contact the University Privacy Officer on 9905 6011.
Terms and Conditions
The following are the terms and conditions for the purchasing and usage of your Monash Community Card:
- Students purchasing a Monash Community Card will have their card and welcome pack sent to their official postal address as registered in WES (Web Enrolment System). If you wish to change your current postal address, please visit the change of address facility in WES. Staff will have the opportunity to specify their postal address when purchasing online. Please allow 7-10 days from date of purchase to receive your card and welcome pack.
- No refunds are available for the Monash Community Card.
- A receipt of purchase for your Monash Community Card will be emailed to your Monash email address. You can print out the receipt of purchase to access on-campus benefits whilst waiting for your Monash Community Card to be delivered.
- The Monash Community Card is valid for use only by the cardholder and is not transferable under any circumstances.
- The Monash Community Card is only valid for the period of use indicated on the card.
- Monash University, the Student Organisations, and the My Rewards Program reserve the right to require the Monash Community Cardholder to produce their Monash Community Card as well as their Monash University Student Card at any time whilst utilizing services or obtaining benefits. Monash Community Cards may need to be scanned to gain access to some services as well as their Monash University Student Card or Staff Card at any time.
- In the case of stolen or lost Monash Community Cards, a replacement card fee of $30 will be charged by the university. As evidence that your Monash Community Card needs to be replaced, you must show your damaged but identifiable Monash Community Card, or a police report or statuatory declaration stating that the card has been lost, damaged or stolen.
