The information on this form is collected for the primary purpose of organising the Peer Mentor Program. Other purposes of collection include communication with you. Your information will be handled in accordance with the University’s privacy procedure. Personal information may also be disclosed to other students participating in the program. If you choose not to provide all the information requested in this form, Monash University may not be able to match you with another student in the program.

You have the right to access personal information Monash University holds about you, subject to any exceptions in relevant legislation.
