Mentor Application Form

Please read this page thoroughly before submitting your application.

Step 1: Compulsory Mentor training is held twice a year - February and July. Training for semester 2, 2009 will be held on Thursday 2 July in the Banquet Room (Building 10, Clayton Campus) from 9.30am to 3.30pm.

Step 2: Fill in application form below. Applications for semester 2, 2009 are due by 30 June 2009.

Step 3: Read and agree to the terms outlined and SUBMIT your form.

Step 4: Wait for your campus Health, Wellbeing and Development (HWD) office to contact you.

Briefly explain why you would like to be a mentor
Write a brief introduction about yourself, this could include the things you like to do, hobbies, interests, what you want your mentee to know about you.

We ask that you agree to:

  • Attend two days of training and campus mentor activities.
  • Ensure regular contact is maintained with the mentee(s) for 6 weeks, and if for some reason this is not possible to inform the coordinator;
  • Provide regular feedback to the coordinator on your activities within the program;
  • Inform the coordinator if there are problems or issues of concern so that these may be addressed as quickly as possible;
  • Recognise personal boundaries and the necessity to exercise discretion in dealing with difficult issues;
  • Respect the culture and beliefs of your mentee. Check with your mentee if there is anything that they will not eat for religious or other reasons and be sensitive that some topics or jokes may be embarrassing or offensive;
  • Remember you are a role model for the mentee, they may not be aware of Australian laws, and it is your responsibility to ensure you always abide by correct rules and regulations
  • Not encourage mentees to indulge in any form of illegal drug use or inappropriate activity;
  • Encourage your mentee to seek assistance from Monash University staff for serious problems and/or academic support. Although you will naturally be concerned for the welfare of your mentee, you are not required to be a student counsellor or academic tutor;
  • Not financially support your student in any way, including offering a loan;
  • Let the Mentor Coordinator know if you are having any problems with your mentee, if you are no longer interested in being in the program or if you have changed your contact details; and
  • Have fun and enjoy the time you spend with your mentee.

Confirmation

  I would like to be introduced to a new student(s) through the Monash Peer Mentor Program. I understand that this program introduces mentors and mentees solely for the purpose of providing transitional support and developing cross-cultural friendships. I will respect the culture, beliefs and customs of my mentee. I have read the above guidelines for Mentors and understand and agree to the responsibilities for mentors outlined therein.

Privacy Collection Statement

The information on this form is collected for the primary purpose of registering your interest in participating in the Monash Peer Mentor Program. Other purposes of collection include: to match you with a mentee, to communicate with you and conduct de-identified statistical analysis. Personal information may be disclosed to the mentee for program administration and matching purposes. If you choose not complete all questions on this form it may not be possible for your application to be assessed by Monash University. You have a right to access personal information that Monash University holds about you, subject to any exceptions in relevant legislation. Monash University reserves the right to not approve the application of any person deemed unsuitable for the Peer Mentor Program.
