MUSHP Enrolment procedures

How to apply

Step one: Complete the enrolment form:

You need your Child Care Benefit (CCB) rate and your Customer Reference Number (CRN) from the Family Assistance Office/Centrelink

Step two: Calculate the fees using the holiday program fee calculator or contact MUSHP. The fee is calculated on the CCB rate. If this rate is not confirmed by Centrelink and/or you do not provide your CRN, the amount may vary.

Step three: Complete the general child profile form (pdf 130kb) if:

Contact the program coordinator to discuss any additional or special needs.

Step five: Pay and return the forms by the closing date:

We give priority to families working and studying at Monash University. Other families are given any remaining vacancies (prioritised by the Government's priority of access).

Late applications

If places are available, we will process applications received after the closing date. Late applications:

Confirmation of enrolment

We will email you a receipt and a Confirmation of Enrolment Pack after we process the applications.

If we cannot meet all your requests, we will phone you to discuss the places available, a refund, etc.

New families will receive a parent information booklet that provides an overview of the program, policies and procedures, and what your child needs to bring.
