Academic Dress Hire FormsAvailable below for download are Academic Dress Hire Forms for Graduation Ceremonies taking place at Clayton and Gippsland campus. These completed forms should only be returned between 4 and 10 days prior to your graduation ceremony. A Staff Academic Dress Hire Form is also available for Staff members taking part in the Academic Procession during Graduation Ceremonies. Once completed these forms can be faxed to the Academic Dress Coordinator on (03) 9905 1028. All forms are provided in Adobe Acrobat (.pdf) format.
